The form is created via LibWizard and maintained by the LI Lead. The form should not require much revision unless something changes drastically. Statistics are generated from these fields, so it is strongly encouraged not to change it unless necessary to keep the statistical variables intact.
A form submission generates an email with all the required information to book a session.
Copies of these forms are kept in the LI Lead's email, sorted by month of presentation for quick reference. As that month passes, they are moved into a historical file. Keeping this long-term is unnecessary as all information is available through LibWizard.
